Default "Most polluted city in America" declaration by EPA in 1969??

I've searched and came up empty so I thought I'd try here.

Can someone tell me which city was declared "most polluted city in
america" by the Environmental Protection Agency (EPA) in 1969?

If possible, could you please provide a source?
